Communal Area Keycard Receipt Form Edit page

By submitting this Communal Area Keycard Receipt form this is an acknowledgement that I am the legal homeowner and I wish to receive a Communal Area Keycard from the Fairview Commons Homeowners Association (“FCHA”). I understand that I must follow all the rules of the communal area for the FCHA. Submission of this form will also supersede all previously submitted forms for this address. I further understand that there is a $100.00 replacement fee if I lose this keycard and a new form will need to be completed. Furthermore, if I loan my keycard to anyone without the prior authorization of the FCHA it will result in the loss of my communal area privileges for a duration determined by the Board of Directors of the FCHA.
