By submitting this transfer form this is an acknowledgment that I, as the owner of the property, am transferring my rights to the communal area of Fairview Commons Homeowners Association (“FCHA”) to the tenant that is currently occupying my property at the address identified below. This transfer will remain active until I notify FCHA of a change of status of tenant. Submission of this form will also supersede all previously submitted forms for this address.
I give permission for my tenant to have full access to my Communal Area Keycard. I am fully aware that by transferring permission to use this keycard I cannot use the communal area at the same time. I understand that I am still responsible for any fees or damages resulting from the behavior of my tenant’ and that of their guest(s) in the communal area. I also acknowledge that I am responsible for my tenant’s violations, including payment of violation assessments, and/or damages that may occur by my tenant and/or their guest(s).
By transferring my Communal Area Keycard rights, I take full responsibility for educating my tenant on the rules of the Communal Areas. These rules are available on FCHA’s website at wwww.fairview-commons.com under the Community Wide Standards tab.
I understand that it is my responsibility to reclaim my communal area access keycard from my tenant upon termination of tenant’s lease or I may be subject to additional penalties and fees.

OWNER’S ACKNOWLEDGMENT OF POSSIBLE FUTURE FEES OF FCHA’S COMMUNAL KEYCARD
The owner acknowledges an administrative transfer fee of $250.00 will be assessed for all new subsequent tenant(s) at the property address. There is also a replacement keycard fee of $100 if a new keycard must be issued for a new tenant.

PLEASE NOTE, the FCHA has no responsibility to your tenant. FCHA communications, including violation notices, will ONLY be forwarded to the owner. It is the responsibility of the Owner to notify tenant of any FCHA communications, including violations. Failure of Owner to notify tenant of FCHA communications, including violations, will not absolve Owner from paying violation assessments. PLEASE DO NOT INSTRUCT YOUR TENANT TO CONTACT FCHA.
